Almost always because the font was chosen for Russian. Macedonian Cyrillic has 31 letters, seven of which Russian does not have — Ѓ, Ѕ, Ј, Љ, Њ, Ќ and Џ. Four of those it shares with Serbian; Ѓ, Ѕ and Ќ are used in no other standard Cyrillic alphabet today, which makes them the first to break. A font tested only on Russian drops or substitutes them, and since the person laying out the page does not read Macedonian, nobody catches it. We check rendering on delivery, and it is worth testing your own templates with a real Macedonian string early.

No, and the reason goes beyond accent. Macedonian has lost the noun case system and places the definite article after the noun — features that separate it clearly from Serbian in particular. The sentence is built differently, so a reader from Belgrade cannot simply adapt as they go. A Macedonian audience hears the difference immediately. You can, and your audience will read it as foreign. Macedonian is written in Cyrillic; romanised Macedonian appears in technical contexts and in transliterated names, but a whole piece of consumer or institutional content set in Latin looks like something produced for outsiders. If the constraint is technical — a system that cannot handle Cyrillic — tell us early and we will look at it, but it is worth solving properly rather than working around.
